Appointments
We know you have many choices when choosing a Podiatrist in Lake Zurich , IL , so we have made requesting an appointment a simple process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible..

Insurance
Our office accepts a variety of different HMO's, PPO's and other health plans. Please call our office to verify acceptance of your insurance carrier.

For New Patients
PREOPERATIVE INSTRUCTIONS:

1. For cases scheduled in the morning, patient is to have nothing to eat or drink after midnight.
2. For cases scheduled after 12:00 noon patient may have clear liquids, (tea, water, apple juice, etc...) up until 7:00 AM.
3. For cases scheduled after 4:00 PM patients may eat light breakfast and drink liquids up until 7:00 AM. After 7:00 AM nothing by mouth.
4. If patient is taking medication such as blood pressure meds, thyroid meds, hormones, etc... may take morning dose with I ounce of water. If patient is taking insulin, please consult with anesthetist.

POSTOPERATIVE INSTRUCTIONS:

1. Patient must have responsible adult to escort him/her home.
2. Activity for the day of surgery once you return home should be limited to bedrest.
3. Diet for the day following surgery and your twilight sedation should begin with full liquids (soups, jello, water, milk, etc...) and progress to a full diet by 2nd day or as tolerated.

PAYMENT FOR ANESTHESIA SERVICES:

1. Our policy is to take assignment of benefits from your insurance company. We will submit our bill directly to your insurance company; thus you will not receive a bill from Lake Anesthesia Associates.
2. To extend the above courtesy to you it is important that on or before the day of surgery you bring in a claim form fully completed and signed. We will also need a copy of your insurance card.
3. Along with your claim form we will ask you to complete an "Insurance Data Sheet". This form will be provided to you in the office.
